STATEMENT OF UNDERSTANDING
- Psychotherapy appointments are reserved for 1 hour.
- I require 24 hours notice to cancel or reschedule an appointment. Otherwise you will be charged for the time reserved for you.
- In accordance with state and federal laws, all information disclosed to a licensed Marriage, Family Therapist is confidential and can only be released with written consent or as may be required by a court order. The law mandates counselors to report required information to appropriate authorities under the following circumstances:
- when the client is thought to be in danger of committing suicide.
- when there is reasonable suspicion of child, dependent, or elder abuse.
- when the client threatens serious harm to someone.
- Therapists keep records of clients’ visits, and may consult with a clinical supervisor in some cases. Please discuss with me any questions you may have about how your records will be handled.
- Payment for your session is due at the beginning of the appointment. This eliminates confusion or rushing at the end of the hour.
- I generally do not bill insurance companies directly. You pay for your appointment at the time of service. I give you a completed insurance form and your insurance company reimburses you.
- Before you end therapy, you agree to schedule a final closure session.
