STATEMENT OF UNDERSTANDING

  1. Psychotherapy appointments are reserved for 1 hour.
  2. I require 24 hours notice to cancel or reschedule an appointment. Otherwise you will be charged for the time reserved for you.
  3. In accordance with state and federal laws, all information disclosed to a licensed Marriage, Family Therapist is confidential and can only be released with written consent or as may be required by a court order. The law mandates counselors to report required information to appropriate authorities under the following circumstances:
  • when the client is thought to be in danger of committing suicide.
  • when there is reasonable suspicion of child, dependent, or elder abuse.
  • when the client threatens serious harm to someone.
  1. Therapists keep records of clients’ visits, and may consult with a clinical supervisor in some cases. Please discuss with me any questions you may have about how your records will be handled.
  2. Payment for your session is due at the beginning of the appointment. This eliminates confusion or rushing at the end of the hour.
  3. I generally do not bill insurance companies directly. You pay for your appointment at the time of service. I give you a completed insurance form and your insurance company reimburses you.
  4. Before you end therapy, you agree to schedule a final closure session.
